Air Ambulance vs Commercial Flight Medical Repatriation
Not every medical repatriation needs an air ambulance — and not every patient can travel on a scheduled flight. Choosing the right solution is a medical decision first, and an operational and financial one second.
When an air ambulance may be appropriate
A dedicated air ambulance is designed for patients who need continuous medical monitoring or intensive care during transport, who cannot sit upright for the duration of the flight, or whose condition makes travel on a commercial aircraft unsuitable. It offers flexibility in timing and routing and allows the medical team to manage the patient in a controlled environment, from bedside to bedside.
When a commercial flight may be feasible
Many patients who are medically stable can be repatriated safely on scheduled commercial flights. Depending on the case, this may involve additional seats, a business-class seat for comfort, oxygen, wheelchair assistance or a stretcher where the airline offers it, and an accompanying medical escort. Airlines require medical clearance, and availability can vary, especially in peak season.
The factors that guide the decision
- Clinical condition and stability — the treating physician’s assessment and the insurer’s medical review.
- Level of care during travel — whether a nurse, doctor or specialised team is required.
- Mobility — seated, assisted or stretcher.
- Timing — how urgently the patient needs to reach the receiving facility.
- Destination and route — distance, connections and receiving-hospital arrangements.
- Cost — balanced against what is medically appropriate.
Medical escorts and ground connections
Whichever option is chosen, ground ambulance connections between the hospital and the airport — in Crete and at the destination — are often part of the plan, along with appropriately qualified medical personnel to accompany the patient when required.
